professional expenses/deductions

I'm an employed physician, getting a W2. I have other career expenses, not specifically related to my W2 job. (e.g.: maintaining licensure in other states). What is the best way to deduct these? Is there any way around the initial 2% of expenses not being deductible?

Can you pay these expenses with pre-tax dollars through your employer? Here is what I wrote in the most recent newsletter available at www.mdtaxes.com/news0215.html:

Business expenses paid through employer's "slush fund": If you have the option of having your employer pay for some of your out-of-pocket expenses in exchange for a reduced bonus or salary, those expenses are paid with pre-tax dollars.
