selection of an entity

Hello there. I am dentist working two part time jobs as an independent contractor. Should I incorporate before the end of the year? will that be of any benefit now? If yes, what kind of entity should I chose and why? I did not pay any quarterly taxes so far. Can I still pay an amount equal to my last year's taxes by january 15? I would appreciate your reply. Thank you.
