Sinai Hospital in Baltimore (my internship for 2000-2001) sent me a letter to authorize them to release my FICA refund. There was a consent form attached that I was required to send. Things to note:
1) If you receive one of those letters, you must fill it out in its entirety and return it within 45 days or it is considered null and void (45 days from the date of the letter you receive).
2) I didnt realize the interest on the FICA refund was taxable.
