Yes, if both of these organizations are having you fill out W-9 forms, it seems they will pay you as an independent contractor. If they gave you a W-4 form instead, they would be paying you as an employee.

When you are paid as an independent contractor, you can claim your "ordinary and necessary" unreimbursed professional expenses directly against that income on a Schedule C (http://www.irs.gov/pub/irs-pdf/f1040sc.pdf).

And if you'll be doing tele-radiology from your home, and you use that part of your home exclusively for your moonlighting and on a regular basis throughout the year, I would suggest that you are eligible to claim the home office deduction. (http://www.irs.gov/pub/irs-pdf/f8829.pdf)

Hi, My office is located outside of Boston. You can find a listing of all the CPA firms affiliated with MDTAXES at www.mdtaxes.com/offices.html. If one of those CPAs has an office in your metropolitan area, I recommend that you work with that firm. I feel you will get better services and advice by working with someone that you can meet with periodically.
